The P3 Pursuit Psychiatric Rehabilitation Program (PRP) works to assist clients with mental health conditions reach a higher level of self-sufficiency. The PRP program assists clients with retrieving and coordinating services and resources in their communities and works to ensure that the client has access to state of the art treatment. Psychiatric Rehabilitation staff provide services within the home and the community of the client. Our staff assist the client with learning and developing improved daily living skills such as personal hygiene, organizational skills, time management skills, nutrition, money management, as well as working towards attaining healthy coping mechanisms.
Our program’s goal is to help youth, adults and families manage their symptoms and establish a harmonious way of life by assisting them with acquiring the education, skills and resources necessary to build self-sufficient prosperous lives. We are here to help maintain and increase the quality of life for youth, adults and families who are in greatest need to reverse the effects of trauma and emotional instability by providing comprehensive behavioral services in the community and home-based settings.
prp for adults (18 and older)
Our program serves adults, ages 18 and up, with severe and persistent mental illness, priority diagnosis. Clients who are actively receiving treatment from a physician or licensed clinician for a mental health diagnosis, and who have or are eligible for medical assistance.
Community Psychiatric Rehabilitation provides flexible services for individuals served. Treatment is focused on building proficiencies in the following areas:

Development of natural supports through increased opportunities for socialization

Connection to community resources

Navigation of government system/community resources (i.e. SSA, DSS, local housing agencies)/Case management

Community advocacy and safety

Life skills training (such as grooming, hygiene, nutrition, dietary planning, budgeting/money management, medication management)

Improvement of crisis management and wellness

Assistance with scheduling and continuing appointments

Coordination of care with other providers (such as therapist, psychiatrists, PCP, substance abuse providers, specialists)
